[1] Take a new file of 500 pixels,300 pixels, of resolution 72 dpi in the RGB mode.

[2] Take the Ellipse Tool.

[3] Now create a new shape with the help of Ellipse Tool as shown image below.

[4] Go to Blending Option select the Inner Shadow apply the following settings.

[5] Do not click ok select the Outer Glow use these settings.

[6] Select the Inner Glow use these settings.

[7] Select the Bevel and Emboss use these settings.

[8] Select the Satin Structure use these settings.

[9] Select the Gradient Overlay use these settings.

[10] Select the Pattern Overlay use these settings.

[11] Your image should be look like this.

[12] Now take the Shape Tool and select the one.

[13] Create a shape and fill with white color.

[14] And now change the Opacity as shown image below.

[15] Your final image should be look like this.
